Project summary:
Dignity & Care: Dementia and Diabetes Aid brings together partners from Alytus District in Lithuania and Pisz County in Poland to improve dementia and diabetes care through cross-border cooperation, shared expertise and better access to healthcare services. The project strengthens local healthcare infrastructure by creating a specialised dementia care unit at Daugai Palliative Care Hospital and developing a diabetic outpatient clinic at Pisz County Hospital, equipped with modern medical facilities. Healthcare professionals and caregivers take part in joint training, workshops and events, sharing knowledge and good practices in dementia and diabetes care. The project also raises awareness among residents, patients, families and caregivers through social campaigns and community activities. Drawing on the partners’ experience, including mobile healthcare services, the project explores ways to reach people living in remote areas and improve access to high-quality care. By combining investments, training, knowledge exchange and community outreach, Dignity & Care strengthens cooperation between Lithuanian and Polish healthcare providers and supports more accessible, inclusive and sustainable care for people affected by dementia and diabetes.
